Families of victims killed in a plane crash involving a Black Hawk helicopter are urging the U.S. Army to address safety failures, citing systemic issues and a lack of accountability.

As we've been reporting, the preliminary report into the Air India Flight 171 crash indicated that both engines on the plane were almost simultaneously flipped from run to cut-off. The Boeing 787 Dreamliner then began to lose thrust and sink down, before crashing in the west Indian city of Ahmedabad.
